Processing of your personal data, such as name, date of birth, address and health details, by Mind Monmouthshire is necessary for the delivery of support. Without this information, Mind Monmouthshire will not be able to provide you with support. We have a legal duty to process your data legally, securely and fairly. 

Your personal details will be shared, with permission, to agencies Mind Monmouthshire signpost or refer you to and will be transmitted in a safe and secure manner. Where Mind Monmouthshire is working under contract with other organisations to provide support, e.g. in a consortium, your data will be shared with the other organisations in the consortium.

Mind Monmouthshire is part of Monmouthshire County Council’s Supporting People Contract and we abide by the terms and conditions set out in their contract. We also provide statistical data to our funders.

Your personal details will not be shared further, unless in relation to safeguarding or other legal obligations. We will never sell or share your personal information with organisations so that they can contact you for any marketing activities.

Your records will be safely stored and retained in line with our Data Protection policy and retention schedule, as long as is reasonable and necessary for the relevant activity, unless we need to retain under another lawful basis.

You have a number of rights in relation to the information including the right of access to information we hold about you and the right to complain if you are unhappy with the way your information is being processed. You have the right to have the data we hold on you erased from our records at any time. If you have given your consent for us to contact you about services, surveys, etc. you may withdraw this consent at any time.
